Transitional Care Nursing
Transitional care or care transition is a medical field designed to ensure continuity of the healthcare even if a person moved from one location to another location or defined as various levels of healthcare within the same place. It is very important for elder patients with multiple chronic disorders and complex therapeutic regimens. During transitional care, nurses should ensure that there should not be any poorer outcomes due to medication errors or due to any kind of errors of communication among the involved healthcare providers and between providers and patients family care providers.
